It is given three to five times at two to six week intervals. The scar flattens and itching settles over several months, which is how long the overgrown tissue takes to reduce. Keeping the interval gives the surrounding skin time to recover and lowers side effects.

Surrounding skin can thin or lose colour, because the drug acts to reduce tissue and, if it spreads beyond the scar, has the same effect on normal skin. It is placed precisely inside the scar to limit that, and the amount is adjusted as the result is followed.

It can recur, so the result is followed over time. Keloids come from constitution and tension at that site, so reducing the tissue does not stop regrowth while those conditions remain. A raised scar does not settle on its own, so injection is the standard approach, and starting early gives better results.
